Unique Student Identifier (USI)

​​Why you need a​​​​​​ USI

A USI allows students to:

• Receive nationally recognised qualifications and statements of attainment
• Access an online record of all VET achievements across different training organisations
• Share training records with employers or further education providers
• Track progress toward career and study pathways 

How to cr​​eate a USI

• It is a simple process to create one by using a Medicare Card, Birth Certificate or Passport. Please follow this link to get started: https://portal.usi.gov.au/student/Usi/Forgotten/Success. 
• Once created, the USI should be recorded and kept in a safe place.
• Alternatively, if you require assistance with creating a USI number, please complete the attached form and ensure every step is completed and signed by yourself and by your Parent/Guardian on Page 2 of the form. 

Providing your USI to Ipswich State ​​High School

After creating a USI, students must provide it to the school so it can be recorded for VET enrolment and reporting purposes.  Students can provide their USI by:

• Entering it during the VET enrolment process, or
• Complete the USI form found here​ and return to us for processing ASAP.  Please ensure this is acknowledged by yourself and Parent/Guardian on Page 2 of the form. 

What if I have forgotten my US​​​I

If you think you have a USI and have forgotten it , please following this link to have it either sent to you via text or email - https://portal.usi.gov.au/student/ForgottenUsi 

Privacy and secu​​rity

Ipswich State High School handles USI information in accordance with the Student Identifiers Act 2014 and relevant privacy legislation.

Student USIs:

• Are securely stored
• Are only used for training, reporting and certification purposes
• Will not be shared without student consent, unless required by law 

Important inform​​ation

• Students must provide a valid USI before any qualification or statement of attainment can be issued
• Delays in providing a USI may result in delays in certification
• It is the student's responsibility to ensure their USI details are accurate​
